Venous Interventions: The Rise of Venous Stenting and Aggressive DVT Treatment

The venous system has recently been gaining exposure as an under-treated area of the human vasculature. New evidence and trends in clinical practice are moving towards increasingly aggressive interventions both for acute and chronic venous disease. A major contributing factor was the 2008 Surgeon General’s Call to Action on DVT and Pulmonary Embolism, but the rise of arterial stents in the venous system has also played a role. 
 
MRG’s new Physician Forum survey investigates the changing dynamics of the venous interventional market with a specific focus on acute DVT treatment, and the treatment of obstructive chronic deep vein disease with venous stents. The survey shows that physicians are becoming increasingly aware and increasingly involved in the venous system, whether they are vascular surgeons, radiologists, or cardiologists. Many challenges remain for both acute and chronic disease, however, including greater awareness among primary care physicians, better clinical data, and improved devices.

    • Specialists
      • Respondents note that a lack of education and treatment guidelines severely limit their treatment of DVT
      • Pharmacomechanical thrombectomy is now almost as frequently performed as cather-directed thrombolysis, despite the fact that only 30% of respondents perform it on a regular basis
      • Respondents are dramatically increasing their venous stenting volumes; they expect a 23% increase in venous stent volumes in 2010 over 2009
    • Primary Care Practitioners (PCPs)
      • Only 6.5% of patients with chronic deep vein disease referred by PCPs received a venous stent 
      • Respondents identified increased education among PCPs as the most important factor that would drive venous stenting
      • 1/3 of respondents were unaware of the practice of venous stenting, and fully 40% do not refer patients for the procedure
  • MRG surveyed 98 US specialists who treat deep vein diseases, and 78 PCPs who refer these patients

      • Vascular Surgeons, Interventional Radiologists, Interventional Cardiologists, and Cardiothoracic Surgeons
      • Average of 12.5 patients with chronic deep vein diseases per month (7.7 of them with DVT)
      • 95% of IRs and VSs performed venous stenting (65% overall)
    • PCPs
      • General Practioners, Internists, Emergency Medicine 
      • Average of 8.5 patients with chronic venous disease each month
  • MRG asked physicians 26 detailed questions such as:
    • What do physicians see as major areas for device innovation?
    • What are the weakness and potential improvements of current devices?
    • How are clinical practices around the treatment and prevention of deep vein thrombosis changing?
    • What will be the future role of venous stenting and pharmacomechanical thrombectomy?
  • The data is analyzed and presented in intuitive, easy-to-read charts in a PowerPoint slide deck, along with detailed commentary by MRG vascular analysts
